Using the search function on the Nursing Home Compare website, identify a skilled nursing facility (SNF) in your zip code that has an Overall Rating of 2 or less stars. Please note that you may need to expand your search option to other zip codes in order to locate this rating.

Once you have identified an SNF, read through the various tabs of information on the website to learn more about this facility:

Under Health Inspections, download the most recent health inspection report (CMS-2567). Take time and review the formatting of this form.
The left column identifies the F-Tag (deficiency) that
was cited, the level of harm caused as a result of this deficiency, and the number of residents affected
The right column labels the regulatory identifying information and a summary of each deficiency
In the discussion, address the following:
Provide a brief overview of your select SNF (i.e. name, location, size, ownership, etc.)
How many stars did the facility get for its health inspection rating, staffing rating, and quality measures rating?
How do this facilitys fire safety standards compare to levels in the state? Nation?
How do this facilitys staffing levels compare to levels in the state? Nation?
Discuss one deficiency that was cited at this facility. You will need to open the CMS-2567 health inspection report to locate this information:
What was the F-tag?
What was the level of harm?
How many residents were affected?
What regulation was violated?
Summarize the violation that was cited
What would you offer as the plan of correction to rectify this violation?
