Conduct a Root Cause Analysis (RCA) for the medical records process
Identify what the problem is, and move forward in identifying the root cause of it. Utilize RCA tools such as the 5 Whys, technique, an Ishikawa cause and effect diagram, etc. in order to identify causality. Identify the parts of the process that can lead to inefficiencies, or poor user experience, or inconsistent results. Discuss how you chose to evaluate the process by determining which baseline data to obtain for the process (example, you chose to track how often a nurse administered medication and documented in the E.H.R. that it was done which indicated a successful workflow vs not documented due to an interruption). Give your opinion on the impact re-engineering would have on the workflow process and the experience of the?user. Include specifically the pros and cons of health information technology as the means by which the change is made.??? Note that you do not need to re-engineer or redesign the workflow process for this assignment, but rather discuss that you have an understanding of how you would approach it (since you understand the root cause of the problem), and why that would be beneficial.
