response to this post for diverticulitis.
Diverticulitis is a condition characterized by an inflammation of the intestine’s diverticular mucosa(Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019). They are pouch-like protrusions in the colon’s sigmoid segments (Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019). There are two types of birth defects: congenital and acquired. Congenital are found in the intestinal wall layers and on the antimesenteric edge of the colon (Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019). Acquired on the gut’s mesenteric edge, where blood vessels invade the bowel wall, usually where arterial arteries perforate the colon. The majority of diverticula are asymptomatic and discovered by chance during a colonoscopy (Goolsby EdD MSN NP-C FAANP FAAN, Mary Jo & Laurie, 2018)
Common signs and symptoms seen
The pain in the left lower quadrant (LLQ) of the abdomen is the most common clinical manifestation. Usually gets worse after a meal (Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019). Bowel patterns range from diarrhea to constipation, and if they rupture, they might cause bleeding (Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019). Bleeding can be significant and painless at times, but if it is severe enough, it can result in a high fatality rate. Fever, chills, and tachycardia will occur if the diverticula become infected (Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019). In some situations, the diverticula can cause fistulas to form between the bladder and the uterus; if this occurs, fecal material can infiltrate the bladder, causing infection and dysuria.
Screening assessment tools
A physical examination may reveal LLQ pain and, if permitted, a solid fixed mass in the diverticula (Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019). Occult blood will be detected in the stool. Mild to moderate leukocytosis can be detected with a lab test. White blood cell counts are normally normal, but if you’re bleeding internally, your hemoglobin will be low (Fischbach RN BSN MSN, Frances Talaska et al., 2017). Urinalysis can be used to check for feces in the urine or infection.
Recommended diagnostic test (If any)
Abdominal x-ray films can be taken, revealing the presence of free air (Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019). If the patient has an ileus or obstruction, abdominal x-rays will reveal it. D

iverticula and sinus tracts of fistulas can be easily defined with barium enemas. A colonoscopy or cystoscope can also be used to check for the formation of diverticula and fistulas (Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019). A CT scan with oral contrast, which displays pouches, fistulas, and obstructive processes, is the most sensitive test (Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019). It would also reveal any aberrant colon wall thickening.
Treatment plans both pharmacologic and non-pharmacologic based on current
clinical practice guidelines
If diverticulitis is discovered by chance, it can be treated with a high-fiber diet or psyllium supplements on a daily basis. If the symptoms are modest, the patient might be treated with a clear liquid diet and rest (Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019). A colon resection of the damaged colon may be performed if someone arrives with acute symptoms and bleeding.
If a patient needs antibiotics, they’ll usually be given “amoxicillin and clavulanate potassium 875/125 mg orally twice a day or metronidazole (Flagyl) 500 mg orally three times a day with trimethoprim/sulfamethoxazole (Bactrim DS) 160/800 mg orally twice a day for 7 to 10 days or until the patient is afebrile for 3 to 5 days The symptoms normally go away fast, and the diet can then be progressed to soft, low-roughage foods, followed by high-fiber foods as tolerated. Antispasmodics such as hyoscyamine (Levsin) 0.125 mg every 4 hours, dicyclomine (Bentyl) 20 to 40 mg four times day, and buspirone (BuSpar) 15 to 30 mg/day can help with spasm pain” (Dunphy et al., 2019). Patients will need to see a gastroenterologist for treatment and will need to have regular screenings and colonoscopies (Dunphy PhD APRN FNP-BC FAAN FAANP, Lynne M. et al., 2019).
To avoid further issues, patients will need to be taught on high fiber diets; however, if they are not already doing so, they will need to start cautiously to avoid bloating and gas, which would make them non-compliant with the new way of eating. They’ll also need to drink more water to help with regular, non-constipated bowel motions. They can take fiber pills if they don’t want to eat more fiber, as long as they drink lots of water to avoid a blockage. They will need to be taught the signs and symptoms of worsening issues, as well as when to seek medical treatment, as with any disease process.
